Anna DeCinque, star of Extreme Sisters, fell into tears and struggled to breathe after realising she does not look exactly like her twin sister.
The TLC show chronicles the lives of sisters who are so close that they defecate together, while others desire to appear identical – down down to their teeth.
In a sneak peek for a forthcoming episode, Anna, dressed identically to her twin Lucy, tells her dentist, ‘Since last time, it’s gotten infected again.
‘It was pretty bad, like excruciating pain, kind of unbearable, like it flared up.’
‘She’s been really upset about it,’ Lucy adds.
Anna falls into tears and needs to leave the room at this moment since it’s all too much for her.


Walking out, she says: ‘I need to breathe, I don’t know what to do,’ while Lucy comes to comfort her and asks: ‘Are you going to have an anxiety attack?’
‘Just breathe,’ she repeats.
‘I don’t know what you’re going to do,’ Anna tries to tell Lucy, through the tears.
Cutting to a confessional after the scene was filmed, Anna explains: ‘Our main fear is what if our mouths are different and we’re not the same.’


She adds, tearfully, that she now understands she’ll have to have her teeth out owing to the agony, but that this will mean the twins are no longer precisely identical.
‘I want to take it out, but I won’t without my sister.’
Cutting to the dentist’s point of view, he adds: ‘They’re quite emotional and particularly when they have an infection, pain and other things.
‘Once you spend time with them, and reassure them for a while, they usually comfort each other and that settles things down.’

He then tells Anna: ‘I know you wanted to be identical, but the main thing is we want to make sure you’re comfortable and your infection is treated.’
Lucy wells up as she listens to the dentist talk to Anna about her discomfort and realises they may not look the same anymore.
Bursting into tears, Lucy later says: ‘I’m really, really worried that we won’t find a solution for the both of us.’
Anna adds: ‘I don’t wish you had an infected tooth, but I wish in some ways we had identical mouths and identical teeth.
‘I do feel alone in my tooth, I really do.’
Captioned alongside the clip, TLC wrote: ‘Twins Anna and Lucy are committed to being *exactly identical* but an infected tooth throws the pair into an identity crisis!’
